First, we need information about the material you'd like to register. Remember, fields with an asterisk ( * ) indicate that the information is required.

The following items apply to the item to be registered:

The following Personal Information items apply to the Registrant

SS# or equivalent is required to complete registration process.

*WGA members please use your WGA Account Number (without hyphens) for registration discount*

* Identification

	Note: Full legal name is required.
* First Name	Registrant: First name is required We can only accept ASCII characters
Middle Name	We can only accept ASCII characters
* Last Name	Registrant: Last name is required We can only accept ASCII characters
* Address Line 1	We can only accept ASCII characters Registrant: Address is required
Address Line 2	We can only accept ASCII characters
* City	Registrant: City is required We can only accept ASCII characters
* State/Province (Required for US/Canada)

If Registrant is not an author, uncheck "Registrant is Author" box and click the "Add Author(s)" button.
